ALL-DISTRICT/ALL-STATE APPLICATION FORM
Carefully read the information below before
filling out this application.

1.	When filling out this form, you will be applying for the IHSTeCA All-District/All-State Team.

2.	All information on this form MUST BE TYPED OR PRINTED NEATLY!

3.	Only designated high school tennis coaches may nominate players. Information should only be from the high school season. Midwest, CITA, and NITA rankings do not count. Information about wins and losses against other 2008 nominees is vitally important for proper selection.

Coaches are required to provide a complete typed or printed listing of the nominee's 2008 Record of the player's season on a PLAYER RECORD SHEET. Include position played, opponent, school, date, score of each match, and win or loss. This PLAYER RECORD SHEET form is available online, also (www.ihsteca.com)

All forms SHOULD be sent to your District Meeting Chairmen (the selection meeting is Wednesday, May 28, 2008). You may bring the completed forms to the meeting, but it is STRONGLY RECOMMENDED that all forms be sent to your District Chairman to make sure the forms are at the meeting in case you miss the meeting. Forms must reach the chairman by Friday, May 23, 2008. Forms brought to the meeting must be filled out completely prior to the start of the meeting!

6.	Please include the Coach's address on this form. If selected, certificates will be mailed directly to the Coach.

7.	If you nominate more than one player, duplicate this form.

8.	*A player must first be selected to the ALL-District Team to be considered for ALL-State.

9.	A player may be nominated for Singles or Doubles but not both.
